Institutional Accountability
When an AI system causes harm - a wrongful denial of benefits, a discriminatory hiring decision, a flawed medical recommendation - someone needs to be answerable. Institutional accountability means the organisation deploying an AI system can't simply point at the technology and shrug. It requires clear internal governance: someone owns the decision to deploy the system, someone monitors its performance, and someone is responsible when things go wrong. This is about more than having an ethics board or publishing a set of principles. It means embedding accountability into operational processes - sign-off procedures for high-risk deployments, escalation paths when issues are detected, and genuine consequences when governance is ignored. Many organisations are establishing AI governance committees, appointing chief AI officers, or creating internal review boards. The challenge is making these structures meaningful rather than performative. A governance board that rubber-stamps every proposal isn't providing oversight. For your organisation, the question is whether your accountability structures have real authority and real teeth - whether they can slow down or stop a deployment that doesn't meet your standards, even when there's commercial pressure to ship.
